GRADUATE'S BIODATA REGISTRATION

Instructions for Biodata Form

1. The student need to upload recent 2inches by 2inches sized passport photograph with white background, without cap and eye glasses.
2. The arrangement of the name must be in this order - first name, middle name and last name. This is the format in which students name will appear on their certificate.
3. After successful submission online, students must print and submit a copy of the Biodata form to Academic Office for final documentation.

Note: Other information on the form must be accurately completed.
